Athletics Registration
Online Registration for all students grades 9-12
Please follow the steps below to register your child for a sport.
Step 1: Log into your Genesis Parent Portal account.
Step 2: Select the “Forms” tab.
Step 3: Complete each of the following forms:
Annual Athletic Forms 2024-2025 (to be completed once per school year)
Winter Sports Registration 2024-2025
Random Drug and Alcohol Form 2024-2025 (to be completed once per school year)
Step 4: Select the answer button to acknowledge each of these forms and your agreement. You must repeat this process and acknowledge each of the above forms for registration.
Pre-Participation Physical Exam
If you have not had a physical within 365 days of the first day of practice, you will need to submit a completed Pre-Participation Physical Exam a minimum of two weeks prior to the first day of practice. If you do not submit this paperwork, you will not be eligible to try out, practice or participate.
Only original copies with the physician's signature and stamp are acceptable. Scanned copies and faxed copies are not acceptable.
Completed Pre-Participation Physical Exams are to be submitted to the school nurse. Pre-Participation Exams are not to be given to coaches.
In the Genesis Parent Portal under the Athletics tab you can see the latest physical exam that is on file for your child.
Helpful Tips to Ensure Your Pre-Participation Physical Exam is Complete
Vital signs must be completed and within normal limits.
Visual Acuity must be completed. Vision must be screened with eyeglasses if prescribed.
The appropriate "Cleared" box must be check by the physician.
"Date of Exam" is the date that the actual physical was performed, not the date the paperwork is completed.
Physician's stamp is required on each page near the Physician's signature.
All Health Care Providers are required to complete the Professional Development Cardiac Assessment before performing any student-athlete's physical examination. COMPLETED CARDIAC ASSESSMENT PROFESSIONAL DEVELOPMENT MODULE must reflect the date that the physician actually took this professional development course, not the date of the physical exam. A copy of the professional development certificate should be attached if available.
All "yes" and "no" items must be answered on the Health History Form. If a response is "yes", an explanation must be given that indicates the issue has been resolved, including the date the issue was resolved. Physician clearance notes may be needed and should be included.
Signatures are required of BOTH the parent and the athlete.
The date of the physical must be within 365 days of the first day of practice/tryouts.
Any special clearance that is necessary must be included before athlete will be cleared such as a cardiology or orthopedic clearance.
Baseline Concussion Testing
Baseline Concussion Testing is required for all freshman, juniors and new registrants participating in athletics. Beginning in with the Fall 2021 season Lenape Valley will be utilizing Sway as our cognitive and balance testing platform. Baseline testing will be performed at school under the direction of Ms. Pollard, our athletic trainer.
To be prepared to test, please download the free “SWAY MEDICAL” app to your device from the Apple or Google Play store prior to the test day (IPHONE and/or IOS USERS MAY NEED PARENT PERMISSIONS TO DOWNLOAD AN APP). You do not need to take any further steps until testing day. No account creation is needed. A session code will be provided before testing. Please make sure your phone is charged prior to coming to practice. Testing takes approximately 20-30 minutes, depending on an individual’s pace.
