Allergy and Anaphylaxis School Care Plan & Orders
Any medication (extra epi-pen to be kept in the nurse's office) should be sent in a labeled Ziploc bag with the student's name, DOB, and grade
Asthma School Care Plan & Orders
Any rescue inhaler or nebulizer supplies should be sent in labeled Ziploc bag with the student's name, DOB, and grade
Diabetes School Care Plan & Orders
Any supplies should be sent in a labeled Ziploc bag or Rubbermaid bin with the student's name, DOB, and grade
Any medications should be sent in a labeled Ziploc bag with the student's name, DOB, and grade
School Medication Administration Form - for any medication not provided by the school Excluding inhalers, EpiPens, diabetic or seizure medications (use the specific forms above)
"Annual Health Office Update Forms" are found in the parent portal in Genesis and must be updated and signed yearly which permits the school nurse to administer: Acetaminophen (Tylenol), Ibuprofen (Advil), Midol, Tums & cough drops.
Prescribed Medication:
As per the State Dept. of Education, any prescribed medication needs to be delivered to the nurse's office by a parent/guardian, in original pharmacy packaging with a label along with a physician's order and parent/guardian written permission (see form above).
Ask your pharmacist for the medication to be divided into two bottles completely labeled- one for home and one for school. Do not send medications in baggies, tissues, envelopes, etc.
